Please complete the Student Enrolment Form or contact the school office for a paper copy.

The following documentation is required with all enrolments:

• Enrolment Form
• Birth Certificate
• Medicare Immunisation Certificate
• Current VISA & Passport (if applicable)
• Any other medical/health documents eg: asthma or anaphylaxis plan
